A Plan of Care is a form that contains individualized information on a student with a prevalent medical condition including: anaphylaxis, asthma, diabetes, epilepsy and other features.
Please return appropriate forms as soon as possible.
Parents/guardians should contact school administration if their child has a new diagnosis of a prevalent medical condition.
